Supporting young people with disability transition from school to adult life

Carers Queensland is working with families and carers of young people aged 12 to 17 years old to identify potential barriers and challenges for young people transitioning from school to adult life, as well as the positive impacts that have supported students during this important life transition.

Carers Queensland will be hosting Family Connect Forums for families and carers to connect and learn from one another, increase their knowledge about successful transitions from school to adult life, and to feel empowered to set and achieve transition-related goals.

Share your thoughts and ideas about how to improve this transition stage to enable young people with disability to build their capacity to confidently move towards having choice and control over their own life journey.

Carers Queensland is working with the National Disability Insurance Scheme (NDIS) to deliver the Local Area Coordination Partner in the Community Program.
