Carers Queensland is working with parents and carers of young people with disability attending Years 9 to 12/13 at high school to identify positive pathways as these students transition from school to adult life.
The Family Connect Forums will help to:
- connect and learn from other parents and carers
- increase your knowledge about successful transitions from school to adult life
- feel empowered to set and achieve transition-related goals with your young person
