Supporting parents and carers to plan for transition from school to adult life

Carers Queensland is working with parents and carers of young people with disability attending Years 9 to 12/13 at high school to identify positive pathways as these students transition from school to adult life.

The Family Connect Forums will help to:

  • connect and learn from other parents and carers
  • increase your knowledge about successful transitions from school to adult life
  • feel empowered to set and achieve transition-related goals with your young person

“The forum was a good opportunity to connect to other families, get information and ideas and ask for help” – Chermside Forum attendee
